A bunch of people working together to support a single individual...a case, if you will.
The term comes up most often in healthcare, especially when dealing with managed care, such as with nursing home patients.
The basic idea is that these patients require different types of care, administered by different specialties, with support offered by different agencies. A case management approach coordinates these efforts. The goal is to figure out what kind of benefits are available, and combine them in the most effective way.
Also, since we're talking about the involvement of insurance companies and HMOs here, part of the goal is to offer the treatment in the most cost-effective way possible as well.
